and two (2) year service commitment. Installation includes fiber connection to house, home internet set up, and smart device connection to wireless home network.
Standard Installation fee of $99.00 will be billed if services are terminated prior to the end of the two-year term.

If AutoPay is not selected, statements are sent on or before the 22^nd of each month. If not paid by end of business day the 10^th, a $20.00 late fee will be incurred. By signing this application for service, the undersigned hereby grants to Allegheny Community Broadband a license, irrevocable for the duration of the membership, to come upon the property of the undersigned upon reasonable advance notice for the purpose of repairing, replacing, maintaining and/or inspecting any equipment owned or installed by Allegheny Community Broadband or Blue Fiber Corp. personnel.
